Valle del Sol’s annual Mom of the Year event honors four women of diverse backgrounds for service to the community and for their exemplary roles of mothers.

Why? Affordable, culturally competent, integrated primary care can be hard to find, especially when you’re under-insured or not insured at all. Funds raised from this event will support Valle del Sol Community Health Clinics and the thousands of families who rely on these clinics to keep them, and their communities, healthy.

Nominations

Do you know a Mom who deserves to be recognized?

Nomination Instructions:

An online nomination process is being used. Before nominating someone, please review the nomination criteria and questions below. Please allow yourself enough time to complete the entire nomination form. It is recommended that you prepare your nomination off-line, and copy/paste your answers in the space provided on the nomination form.

Criteria:

Nomination must be a woman and her service/contributions made as a resident of or greatly impacting Arizona. All nominees will be evaluated and selected based on the following:

1. Role Modeling: Demonstrating a positive role as a mom.
2. Service to Community: Significant, measurable service to others in the nonprofit arena by way of service in all three areas:
a) Board Service
b) Direct Philanthropy
c) Hours of time donated (outside of paid activities as part of one’s employment)
