Our centrally located Substance Use program is licensed by the Arizona State Department of Health Services and accepts both court-ordered and non-court-ordered referrals.

Use the form below to schedule an intake Monday through Friday from 9 a.m. to 4 p.m. We also accept walk-in intakes Monday through Thursday from 10 a.m. to Noon. Important: Please bring your ID and court paperwork.
