Our centrally-located program is licensed by the Arizona State Department of Health Services and offers DUI classes, screenings and assessments.
Use the form below to schedule an intake Monday through Friday from 9 a.m. to 4 p.m. We also accept walk-in intakes Monday through Thursday from 10 a.m. to Noon. Important: Please bring your ID and court paperwork.
