AHCCCS health plan, Blue Cross Blue Shield, has partnered with Valle del Sol Community Health Center and
designated Valle del Sol as your health home.
Who We Are
Valle del Sol is a community-based health center serving individuals and families across Arizona. We provide
whole-person care, meaning we focus on both your physical and emotional well-being — all in one place, with
compassion and respect. Care is provided for newborns, children, adults, and the elderly — every person who
walks through our doors is a welcome patient.
We offer care at multiple convenient locations throughout the Valley, so it’s easier to get the care you need,
close to home.
What We Offer
As your primary care provider, Valle del Sol can support you and your family with:
- Primary medical care (checkups, preventive care, ongoing health needs)
- Behavioral health services (mental health and emotional support)
- Care coordination to help you navigate your health coverage and services
- Telehealth appointments when coming into the clinic is difficult
- Support services for concerns like transportation, costs, or insurance questions
Our goal is to remove barriers and make care accessible, comfortable, and affordable.
