    • Worried about how to get to your appointment? Don’t be. AHCCCS Patients – Contact your health plan 72 hours in advance to arrange a ride. If you do not qualify for transportation from another source, or for urgent situations, call us as we might be able to make other accommodations.

    • Be sure to bring with you:

      • Proof of insurance and valid identification (required).
      • A list of all medicines you take – both prescription and over-the-counter. Bring the medication bottles with you.
      • Any available copies of your medical records.
      • Dates and results of screening procedures (if available).

      If the appointment is for your child:

      • Birth certificate (required).
      • Records of child’s immunizations.
    • This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
  • Patient Portal

    • For those of you who love being online, try our Patient Portal! You can book appointments, renew prescriptions, get test results and more directly from your phone or computer.
  • Billing & Insurance

    • We accept AHCCCS and most major insurance plans, such as Aetna, Blue Cross Blue Shield of Arizona, Cigna, Humana, Care1st, United Healthcare and many more.

      If you do not have insurance, we can make your visits affordable and can assist you in receiving state healthcare if you are eligible
