Prepare for Your First Community Health Appointment

Download and complete your patient forms before your visit to Valle Del Sol Community Health. New patients should complete the registration packet, insurance information form, and medical history form. Returning patients only need to update any changes to personal information or insurance coverage.

All forms are available in English and Spanish. Print and bring completed forms to your appointment, or arrive 30 minutes early to complete them in our office. If you need help with any form, our staff is here to assist you!

Preparing for Your First Visit

Your first appointment goes more smoothly when you bring a few key items with you. Here is what to gather before you arrive:

  • Identification and Insurance: Bring a valid photo ID and your insurance card if you have coverage. We accept most major insurance plans and offer a sliding-fee scale for uninsured patients.
  • Medical Records and Medications: Bring copies of previous medical records, lab results, and screening procedures if available. Bring all medication bottles, both prescription and over-the-counter, so your provider has a complete picture of your current treatment.
  • For Children's Appointments: Bring your child's birth certificate and immunization records. These documents help us maintain accurate health records and ensure your child stays up to date with preventive care.

Your first visit sets the foundation for your ongoing care. We want to understand your health needs, your family situation, and any barriers you face in accessing care.
